Chapter 1091 begins with Admiral Kizaru's incursion already underway at Egghead. The Marine fleet is rattled as the Navy realizes the enemy will not go down quietly. Marines shout for the troops to get ready, warning each other not to let the Pacifistas use their lasers, while the fleet opens fire with cannons. Sentomaru and the Pacifista units defend the island from below, and the battle around the coast intensifies with explosions, smoke, and heavy waves. The chapter title "Sentomaru" appears as the Marine assault and Egghead's automated defenses collide.

The weaponized Sea Beasts tear through the Navy's ships, smashing vessels and throwing Marines into panic. The Marines fire their cannons and shout that the Sea Beasts are attacking, but the machines keep charging through the fleet. When one of the vice admirals touches down, he fights back directly, saying that if the Sea Beasts want a fight he will give them one. He orders them to leave the ships alone and strikes with "Steam Knuckle," blasting a Sea Beast with a heavy steam-powered punch. Around him, other ships are hit, explosions tear across the water, and the Navy struggles to hold formation under Egghead's counterattack.

The scene shifts to the Labophase, where Vegapunk explains the island's defenses and the Pacifista control hierarchy. He says the Marines and Robo-Beasts are fighting at the coast, and it should be clear that Egghead's defenses are nearly impenetrable. However, Sentomaru is waiting below with a squadron of fifty Pacifistas under his command. The hierarchy is shown: the Five Elders rank above Vegapunk and his satellites, Vegapunk ranks above Sentomaru, and Sentomaru ranks above authority chip holders. Edison apologizes that asking Sentomaru to face Kizaru is cruel, but says the dark memories from Sabaody have returned. Usopp notes that Kizaru, the man Stussy warned about, is here and that fighting has already started below. Sentomaru accepts that he will understand if the Straw Hats choose to flee.

Kizaru and Sentomaru clash at high speed. Kizaru tells Sentomaru that he really ought to surrender, then launches his light attack "Yasakani no Magatama." Sentomaru blocks and dodges through the barrage, while Kizaru's beams scatter across the battlefield. The fight is personal as well as strategic: Kizaru speaks to Sentomaru like someone he knows, but he does not hold back. Sentomaru answers by keeping his stance and trying to protect Egghead long enough for Vegapunk and the Straw Hats to escape. The page emphasizes Kizaru's overwhelming speed and range and Sentomaru's stubborn defensive skill.

Sentomaru is battered and breathing hard, but he still thinks of protecting Vegapunk. He says he owes the old man his life and does not care whose side he has to take if it helps him escape. He apologizes to Kizaru, calling him uncle, because he gave his word to Straw Hat Luffy. Kizaru presses forward, saying that after all this time with pirates Sentomaru has become chummy with them. He knocks Sentomaru through the rubble, but Sentomaru keeps trying to fulfill his duty. The page frames Sentomaru's resistance as loyalty to Vegapunk rather than loyalty to pirates, even while Kizaru treats it as a betrayal.

A flashback shows Sentomaru's bond with Vegapunk and Kizaru. As a child, Sentomaru is tough and hungry, saying the villagers are wimps and that he hates them after being kicked out. Vegapunk asks what he would like to buy with money, and Sentomaru answers that all he needs is a job that pays well so he will not get hungry. Kizaru is amused by the child, calling him cute, and Sentomaru asks whether he can be a bodyguard. When Vegapunk asks if he would really take the job, Sentomaru says it would be awesome. In the present, Sentomaru remembers that he thought he did the best he could for Vegapunk.

Sentomaru uses "Ashigara Palm" against Kizaru, striking with a powerful sumo-style blow. Kizaru acknowledges that Sentomaru's guard is not anything to sneeze at, but the admiral still breaks through the defense. A huge impact explodes across the battlefield, scattering debris and shocking the watching Marines. Sentomaru's last stand leaves him damaged and forced back, but he has bought valuable time. The page shows the end of the duel turning in Kizaru's favor while still honoring Sentomaru's toughness and commitment to protecting Vegapunk.

After defeating Sentomaru, Kizaru takes the authority chip and transfers command of the Pacifista to himself, saying the previous commander is unconscious and out of commission. The objective of the Pacifista squadron is successfully changed. On Saturn's ship, the Elder notes that it is a waste but that if forced to choose, the Pacifista are the better assets to secure, and he orders the Marines to keep him updated on all casualties. Kizaru commands, "All Pacifista units, come in," while the battlefield shifts under his control. With Sentomaru down, Egghead's powerful defensive army is no longer on Vegapunk's side.
Even after falling, Sentomaru has already changed the Pacifistas' orders. He tells all Pacifista units to come in and says that there has been a change of plans: protect the naval fleet and send the weaponized Sea Beasts to the ocean floor. The order is carried out, and the Pacifista squadron's objective successfully changes. The machines turn on the Sea Beasts and blast them, protecting the Navy's ships. Saturn watches the cost-benefit shift with cold calculation, deciding the Pacifistas are worth securing even if other losses pile up. Sentomaru's final command changes the balance of the coastal battle, but it also removes Egghead's biggest shield from the Straw Hats' escape plan.
Inside the Labophase, the crew hears that Sentomaru is finished but refuses to waste the time he bought them. Luffy, Bonney, and the others say they cannot afford to dilly-dally and must use the defense system quickly. York taunts them from captivity, reminding everyone that she is a Vegapunk too and that they will not crack the password easily. Vegapunk tells the group to focus because Kizaru is made of light and could pass through the laser barrier at any minute. The Straw Hats understand that the plan has become more urgent: disable the Frontier Dome, move the ship, and escape before Kizaru reaches them.

Vegaforce 01 moves into position with the Sunny as Lilith and the others prepare to carry the ship over to the back of the island. The emergency siren suddenly rings, startling Chopper, who says no one should be able to get through the barrier. On the island below, Kizaru has indeed infiltrated the dome. He says the Straw Hat Pirates are nostalgic and that the person before him, possibly Vegapunk, almost wiped him out in the past. He then moves toward the power station, Punk Records, and York, noting that those are the three things that cannot become collateral damage. The contrast is sharp: the Straw Hats are trying to launch the Sunny, while Kizaru calmly prioritizes the government's protected assets.

Kizaru states that Dr. Vegapunk's elimination is the top priority. The alarm spreads through the Labophase: Kizaru has infiltrated the dome, and the group warns Luffy to hurry because he and the others are still making their way back. Vegapunk says it would be pointless to lift the barrier if the ship is not yet in position. As the crew scrambles, Lucci suddenly launches an attack at Vegapunk Stella. Stussy reacts first and throws herself into the blow, taking the strike meant for Stella. Sanji and Nami are shocked as Stussy falls, and the page turns the escape crisis into an internal ambush at the worst possible moment.

Rob Lucci stands over the room after striking Stussy, while Sanji cries out to her. Lucci tells Stussy that her friend left her out of the loop, implying that Kaku did not tell her what Lucci was going to do. Sanji quickly borrows Nami's bubble gun and asks what the strange weapon is as he prepares to counter. The sudden betrayal forces the Straw Hats to fight inside the room while Kizaru is already entering from outside. Nami watches in alarm, Sanji arms himself with the unfamiliar bubble weapon, and Lucci's attack confirms that CP0 remains an immediate threat even after the temporary truce.

Zoro intercepts Lucci with a powerful kick and tells him it looks like he waited for backup to grow a spine. Lucci declares that the Straw Hats are as good as dead now that Kizaru and he are working together, but Zoro answers that Lucci should know better than to underestimate them. Their clash crashes through the room and out into the battlefield, beginning Zoro's fight with Lucci. Outside, the others hurry with Vegaforce 01 while someone asks what to do because an admiral has been sent at Luffy. Kizaru, seeing the movement, says he wondered where the ship went when he did not see it at the shore. The crew's escape route has been discovered.

Kizaru spots the Straw Hats and greets Luffy with "Heya... long time no see." Luffy meets him head-on, punching into the admiral's guard as the two clash. Kizaru notes that Luffy is as cheery as ever, but Luffy answers that they are a hundred times stronger than they were two years ago. The page closes the chapter with Luffy engaging an admiral directly, contrasting this moment with the crew's helplessness at Sabaody. As the crew continues trying to move the Sunny, Luffy stands between Kizaru and the escape plan. The final caption emphasizes the stakes: engaging an admiral, now stronger than they were two years ago. Break next week.
