The chapter opens as the Birdcage closes in on Dressrosa. Citizens scream, "Run! Make for the central districts!" and "Waaaaaah!" while the line of death draws ever nearer. Buildings are not simply being surrounded; the cage is dragging them along and cutting them to pieces. The title appears: CHAPTER 784: GEAR FOURTH. On the plateau where the royal palace is located, the group realizes the true horror of the Birdcage. Bartolomeo says this is bad and tells Roger to get the collapsed people up on their feet because they cannot sit and wait for death.
At the former royal plateau, Viola explains the shrinking Birdcage. The palace now stands where Doflamingo is, and if the fight goes on much longer the plateau will be ripped to shreds. She notes that the center of the cage is slowly shifting and that the epicenter of the shrinking is most likely the plateau where Doflamingo stands. As people hurry and shout, Zoro is asked where he is going. He answers with blunt certainty: "I told you! We're gonna stop that thing."
Before the palace sunflower fields, the wounded Law is tended to. Someone says the dwarves really surprised them. Leo or the healers hope Law's arm attaches properly and explain, "If blood starts flowing through it, it'll start to heal." Law answers, "I know. I'm a doctor." Cavendish reflects on what he realized while underground in the trading port: assuming Luffy defeats Doflamingo, the ripples from that splash will be felt the world over. Law is told that all of them will become the eye of the storm, and Cavendish says, "Yeah. That's what we're after."

At the palace battlefield, Doflamingo stares at Luffy and asks, "Gear Fourth?" Luffy drops his sandals, bites into his arm, and begins to inflate himself with a loud breath. He uses "Muscle Balloon!" and expands into a massive, Haki-coated form. Doflamingo watches in silence as Luffy's body swells, his arms and torso darkening and stretching with steam swirling around him. This is the beginning of the new transformation that Luffy declared at the end of the previous chapter.

Doflamingo laughs that this is not even funny anymore and says he thought Luffy had some crazy ace up his sleeve, but all he did was use rubber's elasticity to get bouncier. He asks, "Just how desperate are you?" Luffy explains that for two years he fought wild beasts as big as monsters, and to pound them into submission this size and elasticity were necessary. He says, "I ain't gonna let you have your way anymore." Then he names the form: "Gear Fourth! Bound Man!" A note clarifies that "Bound" means bounce.
